Primary Care Team

The Primary Care LEAD Team has been addressing the prevention and reduction of childhood obesity using the 5-2-1-0 prescription program. This program provides simple steps children can take to achieve and maintain a healthy weight by developing healthier habits that will last a lifetime. The program launched in January of 2017 and has gained such momentum that the Team extended their plans through 2018.

 

YEAR ONE SUCCESSES:

A total of 40 pediatric providers,  including pediatricians and pediatric  ARNPs participated, representing 5  practices and 12 locations across Polk  County.

A total of 2,236 prescriptions were disseminated and 3,999 goals were selected by patients.

We would especially like to recognize  the two practices who disseminated  over 97% of the prescriptions (2,179  prescriptions):

  • Central Florida Health Care (1,016  prescriptions)
  • Lakeside Pediatrics (1,163  prescriptions)

The most popular goal selected was eating 5 servings of fruits and vegetables each day.

  • This goal was selected 1276 times.

The least popular goal selected was getting 1 hour or more of physical activity.

  • This goal was selected 816 times

 

ANNOUNCING 5-2-1-0 PARTNERSHIP  WITH FLORIDA SOUTHERN COLLEGE

New in 2018 is the partnership with the  Florida Southern College School of Nursing and Health Sciences. Nurse practitioner students will be instructed in the use and benefits of the 5-2-1-0 prescription tool.

Students will then be given the opportunity to  utilize the tool with pediatric patients during  practicum rotations as well as introduce

use of the prescription strategy to pediatric providers with whom they do their clinical rotations.

 

The 5210 Prescription for Healthy Habits gives the child the choice as to what healthy habit they want to commit to. They have the everyday healthy habit choice of eating at least 5 fruits or vegitables, reduce screen time to 2 hours or less. Get 1 hour of physical activity, or consume 0 sugary drinks. This program allows health providers to conversate with children and their parents about the importance of living a healthy lifestye.
